HAF at YMCA Lancaster

Working in partnership with Lancashire and Lancaster Council, YMCA Lancaster are running a free holiday club programme for children on free school meals this summer. Come and enjoy fun activities, healthy meals and more!

Funded by the Department for Education, the Holiday Activity and Food programme (HAF) provides free meals and fun activities for children and young people in Lancaster who are eligible for benefits- related free school meals.

The sessions will include a variety of fun activities alongside a nutritious meal each day. They are a great opportunity for young people to get out and about, get active and meet new friends. Click here for an overview of activities. Please note, this may be subject to change


Booking

Places on each club need to be booked.

Bookings can be made by completing the below form. Or, alternatively you can request a pack by calling 01524 32737 or contact us here and selecting the HAF Holiday Club Option.


This form must be completed by all participants and returned to the YMCA Staff prior to taking part in any activities.

Under 16's must have the form verified by a parent/ guardian/carer and have the parental consent for completed prior to commencement of activities.

All information will be kept confidential to the YMCA. The only people who will see this information are the YMCA staff.




A PARQ is designed to inform the Youth Worker coach or instructor at the YMCA if you are aware of any problems that could reduce your ability to take part in an activity

If you have answered YES to any of these questions, please consult your doctor, either by phone or in person BEFORE you start any physical activity and BEFORE you have a physical appraisal with the YMCA. Explain you have taken a PARQ and tell your doctor which question(s) you have answered YES.

Just because you have ticked yes in one of these boxes, does not mean you cannot take part in physical activity. Please seek a doctor’s opinion to the amount of exercise you can safely take part in. This might mean doing a different exercise altogether, or simplifying a target exercise, and building it up as you progress.

APPROVAL - I accept there is an inherent risk of injury in participation of adventurous activities, gym use, external exercise and movement classes. Risk can be reduced to acceptable levels by implementing appropriate risk assessments. Copies of written risk assessments are available on request from the association.

I understand that if desired it is my responsibility to take out personal accident insurance for myself/ my child

Photographs and Recording

During the project staff will take picture and/or record video. These videos may be used to promote the project either on our social media pages, on our websites or in any YMCA publications. If you consent to your child having their pictures taken please tick below.

Authorisation

I understand that by checking the 'I agree', I hereby authorise relevant information to be shared with other organisations and request that you provide any information that may be required by Lancaster and District YMCA, in addition to being photographed and/or video recorded:

In line with new GDPR laws some information we collect may be stored and kept on our files. None of the information you provide will be shared and will be treated with confidentiality. If you agree to us collecting and storing your/your child’s data please sign below
